In Vitro Fertilization (IVF)

In vitro fertilization (IVF) is a specialized fertility treatment that helps individuals and couples achieve pregnancy when natural conception is challenging. The process begins with ovarian stimulation, where the patient takes hormone medications to encourage the development of multiple eggs. During this phase, careful monitoring through blood tests and ultrasounds ensures optimal egg growth. Once the follicles reach the right size, a trigger shot is administered to finalize egg maturation in preparation for retrieval.

The next step is egg retrieval, a minor surgical procedure performed under sedation. Using ultrasound guidance, a thin needle is inserted into the ovaries to collect mature eggs. These eggs are then fertilized in the laboratory with sperm, either through conventional insemination or intracytoplasmic sperm injection (ICSI), where a single sperm is injected directly into an egg. The fertilized eggs, now embryos, are cultured in a controlled environment for several days to monitor their development.

Embryo transfer is a crucial stage in the IVF process. Once the embryos reach an optimal stage of growth, one or more high-quality embryos are selected and transferred into the uterus using a thin catheter. This is a quick and painless procedure, similar to a Pap smear. Any remaining healthy embryos can be frozen for future use. About 10–14 days after the transfer, a blood test is performed to determine if implantation was successful and pregnancy has occurred.

Preimplantation Genetic Testing (PGT)

Preimplantation Genetic Testing (PGT) is a procedure that identifies specific inherited abnormalities and can be carried out for both inherited genetic diseases and chromosome rearrangements. Conditions such as Tay-Sachs, Cystic Fibrosis, Spinal Muscular Atrophy, and Fragile X can be assessed through this testing. When you use PGT in conjunction with in vitro fertilization (IVF), it may lower the likelihood that your child will inherit a genetic disease for which you are at a high risk.

Previously known as Preimplantation Genetic Screening (PGS), testing for aneuploidies—abnormal numbers of chromosomes in an embryo—is now called Preimplantation Genetic Testing for Aneuploidies (PGT-A). Conversely, because Preimplantation Genetic Diagnosis (PGD) specifically targets single-gene defects, it is now referred to as Preimplantation Genetic Testing for Monogenic Disorders (PGT-M).

Overview of the PGT Process:
  • PGT can easily be incorporated into your IVF treatment. The process begins with ovarian stimulation and monitoring. Once the eggs are retrieved, they are fertilized in the IVF laboratory. After fertilization, a few cells are removed from each embryo.
  • An embryo biopsy occurs on day 5 or 6 of the embryo’s development, where cells are extracted and analyzed. Each cell contains all 24 chromosomes within its nucleus, allowing for an evaluation of all 23 pairs of chromosomes.
  • Samples are sent to an external laboratory, which provides a report to your clinical team to be discussed for embryo transfer.
